I watched the entire thing with David and Caroline. After sleeping on it and letting it process, here's some initial thoughts.
I'd like to point out that President Trump didn't shake VP Pence's hand either. I don't blame him one bit for not shaking Pelosi's hand. More on that in a bit.
When Pelosi ripped up the speech, it took my brain a minute to process what I had just seen because it had no frame of reference to draw on. Imagine the outrage if Paul Ryan had stood up while President Obama was still speaking and started ripping up his speech. There would be a national call for him to be removed as Speaker and rightfully so. It was incredibly disrespectful, petty, and horrible optics. It served her radical base and will do nothing to move independent and middle-of-the-road Americans toward the Democrats. However great she might have felt doing it, it did not help their cause for November in any substantial way.
I told David I don't ever remember a Speaker who had to keep her minions under control the way Pelosi has had to deal with her group these past two years. It's appalling.
President Trump didn't bring up Impeachment or the Iowa Caucuses which was wise. He had nothing to gain by doing so. He was deliberate, even, and focused on accomplishments and goals. It was excellent.
The way he called out the 130 members of this Congress who support or who have voted to support (insert radical idea here) was brilliant. It was about policy and drawing a line in the sand in terms of what they want and what he wants to accomplish.
Seeing the Republicans stand up and chant, "Four more years!" was unreal considering many of those people were never Trumpers when he ran, would not work with him during the first year or two due to all the negative propaganda against him, etc.
Re: not shaking Pelosi's hand. I'm glad. I truly am. I'm a Conservative and I've been waiting my entire life for a President to come in, take on these people, stand up to them, and beat them at their own game. President Trump has exposed the people in D.C. who are anti-American and self-serving. I am glad.
As I find good stuff, I'll add it in the comments below.
What did you think of the speech?

Observations from Australia. This is a good summary.

I missed the first half. For the rest, it looked to me like he was hammering the Democrats over and over on the human costs of their policies.
He also had moments of showmanship, but he is not only a showman.
"The best is yet to come" is a very strong slogan.

Bringing in the democratically elected president of Venezuela was brilliant.
Your description of the human costs is excellent. He put faces on so many of these issues.
